All affiliate cards run based off of the fiscal year and will expire June 30th each year. There is a $15 fee payable by cash or check each fiscal year in order to stay up to date. Renewals are sent out in June, and you have the option to mail the completed form and payment back in or you can drop it off during the walk-in hours listed on our main page. If you allow your affiliation to lapse for longer than a full fiscal year, you will need to come to the office in person to complete the process to again be active as an affiliate in the ISU database. If your record does lapse, then you will need to bring all required documents in again to become active in the system. The requirements for each card type are listed below. Please be sure you have all of the required documents before coming to the office.
Non-ISU Employees
***If you have a summer group you will be bringing to our office in May or June you must schedule a time with us prior to arriving. Failure to schedule a time for your group may result in being turned away and having to arrange a time to come back at a later date. Time slots Monday - Thursday start at 11:00 am and run through 3:30 pm depending on the size of your group. This form will need to be filled out completely for each individual and turned in to our office with a minimum of 24 hours before your group arrives to receive their cards. Typed forms are highly preferred to handwritten forms to maintain data accuracy. ISU ID numbers will not be released to anyone before-hand for any reason.***

The non-ISU employee and the sponsoring department must complete the Non-ISU Employee Database Input Form. The sponsor for an affiliate must be on University payroll. A State or Federal issued photo ID is required such as a Passport, Drivers License, Non-Driver Photo ID, Learners Permit, Military ID, or Tribal Card. The photo ID must be non-expired. International affiliates must check in with the International Students and Scholars Office before obtaining the ISU Card. There is a $15 fee to obtain and maintain the affiliate status each fiscal year. A department worktag, cash, or check are the accepted payment options. Maximum expiration date for this card is currently June 30, 2025. If your affiliate is leaving before then, please provide that date on the termination date for cardholder line.
Spouses, Dependents, and Domestic Partners
These individuals are eligible for ISU Cards. The form below must be completed. Proof of association must be presented in order to obtain a card. Marriage certificate, birth certificate, tax return, health insurance document stating both parties are on the same policy, or documentation of declaration of partnership are the accepted forms of proof. A State or Federal issued photo ID is required such as a Passport, Driver's License, Non-Driver Photo ID, Learners Permit, Military ID, or Tribal Card. The photo ID must be non-expired. There is a $15 fee to obtain and maintain the affiliate status each fiscal year. Cash or check are the accepted payment options. Expiration date for this card is currently June 30, 2025.
Alumni Association
Members are eligible for ISU Cards. The ISU Alumni Association Member ISU Card Form and proof of membership to the Alumni Association must be presented in order to obtain an ISU Card. A State or Federal issued photo ID is required such as a Passport, Driver's License, Non-Driver Photo ID, Learners Permit, Military ID, or Tribal Card. The photo ID must be non-expired. There is a $15 fee to obtain and maintain the affiliate status each fiscal year. Cash or check are the accepted payment options. Expiration date for this card is currently June 30, 2025.
