Lost/Stolen Card

Replacement ISU Cards can be purchased at the ISU Card Office in 0530 Beardshear Hall during walk in hours. There is a quick application form on the table right outside our office door you will need to fill out and bring in to request a new ISU Card. There is a $25 replacement fee payable using cash, check, or U-Bill. Old cards cannot be reactivated. Appointments and preorders are not accepted.

To report a card lost or stolen, log in to your Workday account. Click on ‘Menu’ on the top left corner of your screen and select ‘Requests’ from the list. Next you’ll want to select ‘Create Request’ under the actions box. A window will pop up asking for a request type. You’ll type in the white box ‘ID Card Lost or Stolen Card’ and click ‘ok’. Select the box that best represents the card you lost and ‘Submit’. You do not need to enter a comment but can if you’d like. 
If a card is reported as lost or stolen on our website, there is a $25 fee for replacement. A card that has been reported lost or stolen cannot be reactivated if found.

Students 

There is a $25 replacement fee for a lost or stolen card. This fee can be paid with cash, check, or charged to the student's U-Bill. The ISU Card office employee will determine if a new photo is needed or if the photo on file will be used. If the card is replaced or a new card is received the old card will be cut and destroyed. Only one ISU Card will work at a time and all old versions of the ISU Card will never work again after being replaced. Employees 

There is a $25 replacement fee for a lost or stolen card. This fee can be paid with cash, check, or charged to the employee's U-Bill. If the lost card was printed before May 2009 and the individual was issued a Proximity Card, the Proximity card must also be returned. The ISU Card office employee will determine if a new photo is needed or if the photo on file will be used. 

Affiliates 

There is a $25 replacement for a lost or stolen card. This fee can be paid with cash or check. The ISU Card employee will determine if a new photo is needed or if the photo on file will be used.

Student Workday Photo's

Student's ISUCard photo is now being used for the Workday profile photo. If you would like to update your photo on file there is a $25 fee. You will need to bring in your current ISUCard to swap out for the new card matching your profile. The new photo should update overnight in Workday. The fee is payable using cash, check, or Ubill. 

Damaged Cards

Bring your damaged card into the ISUCard Office for evaluation. Damaged card replacements may require a $25 replacement fee if the card has non normal wear, has been intentionally damaged, or improperly stored. If a free replacement is granted, the photo on file will be used. Card must be the current version to be eligible for a free replacement. If the current card was printed before May 2009 and the individual was issued a white Proximity Card, the Proximity card must also be returned. 

Found Cards

Please return found cards to the ISU Card Office located in 0530 Beardshear Hall. Individuals will be notified if a valid ISU Card is found.
